Learning from Regional and National SAR cases

The Safeguarding Adults Review Sub-Group considers regional and national cases and applies and shares the learning within their own organisations. These summaries provide a brief overview of the case and what learning should be considered for the Tees area.

  • March 2022 – Leicestershire & Rutland; Anna – themes around mental health and substance misuse, and those whose capacity to make decisions is consequently affected by their dependency. Richmond and Wandsworth; Evelyn – themes around cross boundary information sharing, Lasting Power of Attorney declining interventions on the person’s behalf, neglect/coercive control, repeated hospital admissions, lack of professional curiosity/challenge.
  • November 2021 – Norfolk SAB; Joanna, Jon and Ben – themes around private hospital settings
  • September 2021 – Sandwell SAB; Anne – themes around Self-Neglect and lack of a multi-agency approach.
